Licensed assisted living facilities, enhanced services facilities, and adult family homes in the State of Washington are required to have “a posting of names, addresses, and telephone numbers of the state survey and certification agency, the state licensure office, the state ombudsmen program, and the protection and advocacy systems.”  (RCW 70.129.030 Notice of rights and services — admission of individuals.)

As the state’s federally mandated protection and advocacy system, Disability Rights Washington provides the following poster to display in your licensed assisted living facility, enhanced services facility, or adult family home. We cannot provide any information on other licensing requirements.

If your facility is licensed as an assisted living facility, enhanced services facility, or adult family home, or is in the process of obtaining a license, here is a copy of the poster that you may print out or have printed to display in your facility.

If your facility is not licensed as an assisted living facility, enhanced services facility, or adult family home, you may post this alternative poster to be displayed at your facility: Here is a copy of the poster that you may print out, or have printed to display in your facility.

If you have trouble accessing the above PDFs or need an alternate format, please email: info@dr-wa.org.
