Long-Term Care Facility Poster

Licensed adult family homes and assisted living facilities in the State of Washington are required to have “a posting of names, addresses, and telephone numbers of the state survey and certification agency, the state licensure office, the state ombudsmen program, and the protection and advocacy systems.”  (RCW 70.129.030 Notice of rights and services — admission of individuals.)

As the state’s federally mandated protection and advocacy system, Disability Rights Washington provides the following poster to display in your licensed adult family home. We cannot provide any information on the other licensing requirements.
